How often does an OT need to supervise an OTA?

It depends entirely on your state board, and sometimes on the OTA’s experience and setting. The form has a field to record your required frequency so the log is measured against the right standard.

Does remote or video supervision count?

In many states some supervision may occur remotely, but the rules vary and change. The log records contact type — in person, telephone, video, co-treatment, or chart review — so you can show exactly how each contact happened.
