Form FAQ
How often does an OT need to supervise an OTA?
It depends entirely on your state board, and sometimes on the OTA’s experience and setting. The form has a field to record your required frequency so the log is measured against the right standard.
Does remote or video supervision count?
In many states some supervision may occur remotely, but the rules vary and change. The log records contact type — in person, telephone, video, co-treatment, or chart review — so you can show exactly how each contact happened.
What should each log entry include?
Date, contact type, duration, and what was actually discussed — specific clients, documentation, or competencies. Vague entries are the ones that cause problems in an audit.
Who should keep the completed log?
Usually both the OTA and the supervising OT retain a copy, with the clinic keeping the file copy. Check what your state board requires for retention.
Will this satisfy an audit?
It captures what most boards ask for, but requirements differ by state. Verify against your board’s rules and adapt the fields before relying on it.
